I would look at the early keeps, Canterbury for example, the normal was basement floor access via a trap door, then using a turnpike stair for access to the other floors, though others like Scarborough had full access from the basement floor, some keeps had straight stairs within there walls the early Welsh Princes towers are mainly that build, there's no hard and fast rule, unless your building to scale and to plan.
If your asking how access to turrets was gained, they were open back with wooden ladders. I've a few ground plans if that would help?
